In order to get the best warranty and repair services please always fill in this FORM and attach a copy to the unit. This FORM is intended to be typed on line and "saved as" for further e-mail attachment. A typed document is always readable by anyone while writting might be difficult to read by foreign people.
Make sure you have collected the relevant data (Hours and landings) of the equipment and of the airplane.
Do your best to type the symptoms that led you to the removal: they are the only input for technicians. Without accurate information, you are sure to deteriorate the efficiency of this process.
